Place du forage ovarien dans le traitement de l’infertilité liée à la dystrophie ovarienne

Résumé

Prérequis : la dystrophie ovarienne est la première cause endocrinienne d’infertilité féminine en Tunisie
But : Préciser l’apport du forage ovarien par voie coelioscopique dans le traitement de l’infertilité par anovulation après échec de l’induction par citrate de clomifène chez les patientes atteintes de syndrome des ovaires polykystiques.
Méthodes : Etude rétrospective allant du 1er janvier 2003 au 31 décembre 2006 portant sur 23 patientes ayant une infertilité liée à des ovaires polykystiques et qui ont bénéficié dans le cadre de leur hypofertilité d’un forage ovarien par coelioscopie, au service de gynécologie-obstétrique de l’hôpital Mahmoud EL Matri, Ariana, Tunisie.
Résultats : Parmi les 23 patientes ayant eu le forage pour résistance au citrate de clomifène, nous avons obtenu neuf grossesses. L’Age moyen des patientes ayant eu une grossesse après un forage ovarien était de 29.6 ans; le délai moyen était de 7.1mois. Concernant les facteurs pronostiques, seule une durée de stérilité inferieure à 3ans était prédictive d’un bon résultat.
Conclusion : Le forage ovarien présente une alternative intéressante au passage direct en fecondation in vitro chez les patientes porteuses d’ovaires polykystiques.
